Malaise. Abnormal LFT’s
Course shrunken liver on Ultra Sound Scan. ? Cirrhosis. No alcohol
NATURE OF OPERATION/PROCEDURE
Ultra Sound guided liver biopsy
MACRO DESCRIPTION
Single tan and green core measuring 16mm and a 3mm pale and green fragment. (1 block, all embedded).
MICROSCOPY
Liver core showing brad bands of fibrosis containing a mild chronic inflammatory cell infiltrate. Bile ductular proliferation is seen within fibrosis, but this is not marked, and there is no ductopaenia, periductal fibrosis, or cholangitis. There is mild focal interface hepatitis and very focal chronic lobular, inflammation and focal cholestasis. No evidence of steatosis, Mallory’s hyaline, haemochromatosis or alpha-1-antitrypsin bodies is seen, and there is no marked accumulation of copper-associated protein. Rubeanic acid staining shows no definite intracellular copper. There us no evidence of dysplasia and malignancy.
The overall appearances are most in keeping with an established macronodular cirrhosis (rather than congenital hepatic fibrosis). The aetiology of the cirrhosis is not apparent.
